A previously-untried two-page perforated ballot design that generated widespread scanning-machine jams and a massive midterm voter turnout caused the city’s Election Day meltdown, according to Michael Ryan, the Executive Director of the city Board of Elections.
Ironically, the perforated ballot requirement is a state mandate that dates back to when New York used the mechanical lever machines, which it discontinued after 2009.
